First, create a secure MyChart account by downloading the app or going online. (To protect your health information, you will be asked some questions to verify your identity- i.e. vehicle registration, previous addresses, etc.) This account will allow you to cancel or reschedule your appointment, message the Wellness Team and your Primary Care Provider and view your blood results.

Second, HSD Wellness Appointment Scheduling In MyChart SPH so you can select your school of choice and appointment time.

You only need to schedule a flu shot if that’s the only service you need. If bringing dependents for a flu shot, you do NOT need to make them an appointment. Flu shots are available as a walk-in.

Finally, don’t forget this includes a FASTING BLOOD DRAW. You will need to fast for 12 hours prior to your appointment. Please drink lots of water and avoid alcohol for 24 hrs. You can take regular medications with water.
